A flash fire killed three astronauts during a ground test. Pure oxygen atmosphere. Faulty wiring. Sealed hatch. It was devastating. NASA completely redesigned the Apollo command module. Two years later, Apollo 11 landed on the moon. Those astronauts didn't die in vain.
